Responsible for ensuring effective delivery of client services in accordance with Canadian Red Cross’ mission statement, established policies and procedures, relevant legislation, government guidelines, the Collective Agreement and the LHIN MSAA.

Provides supervisory support to Community Support Workers and actively engages the CSW’s as key members of the circle of care. 

General Responsibilities and Activities

  • Ensures the establishments of an effective plan of service / care which is consistent with MOH Policy and the LHIN MSAA, utilizing the InterRAI CHA assessment tool and includes the client and caregiver in care and service planning
  • Completes InterRAI CHA assessments based on outcome scales and CCIM best practices, and as needed for each client (i.e. when the client’s or caregiver’s status or health condition changes), ensuring the Client Assessment Protocols (CAPS) inform changes in the client’s care plan, including client and caregiver input.
  • Responds to and follows through on urgent situations regarding clients and CSWs
  • Ensures that CSWs are informed in a timely manner of any potential or real risks within the scope of their position
  • Actively engages the CSWs in the development / revision of the care plan, recognizing that they are integral members of the client’s circle of care
  • Monitors HPG daily for client referrals
  • Collaborates with CCAC to determine new client admissions to the program; receives, reviews referrals, and assessments from CCAC, ensuring the established inter-agency business process are being followed
  • Investigates incidents and fall events, completes Incident and Fall Events Reports, develops and implements prevention plans and modifies the service plan as required
  • Ensures compliance with all Quality Management policies and Risk Management Procedures
  • Monitor client and caregiver satisfaction with programming and activities
  • Maintains current, accurate comprehensive documentation on all client records (in-home, electronic, office); maintains current and accurate CSW documentation (electronic. office)
  • Ensures meals meet client special diet requirements and requirements of Health Canada’s Food Guide
  • Ensures safe food handling and storage practices are followed
  • Ensures provision of client personal support requirements from the individual’s care plan
  • Ensure safe client medication storage
  • Develops CSW service schedules
  • Informs CSWs of assignments and changes to their schedules
  • Enters all relevant client and CSW information into the automated information system
  • Shares concerns related to the client’s medical condition, behaviours, social situations with caregivers
  • Monitors caregivers status (caregiver stress / feeling overwhelmed, needing more help, illness)
  • Develops effective working relationships and communications with other health / social service agencies
  • Review, implements, and complies with all policies and procedures as directed
  • Generates reports and performs statistical analysis as required
  • Assists the Manager by providing narratives around statistical data for LHIN reports and effective management as required

Human Resources

  • Trains and mentors staff on the implementation
  • Completes the recruitment and selection process for staff and volunteers and conducts orientation, training and In-Service training for CSWs
  • Monitors CSW performance by on site evaluation, internal / external feedback and client / caregiver feedback
  • Assesses CSW performance through the completion of probationary reviews and annual performance reviews
  • Receives and responds to CSW work-related concerns / issues in a timely manner
  • Responds to CSW performance concerns in a timely manner through investigation and follow-up as required, with an appropriate Management or Human Resources Representative.
  • Ensures compliance with the Collective Agreement, as applicable
  • Ensures that the employee recognition program is maintained for CSWs
  • Monitors CSW ability to effectively implement the activity schedule and mentors as needed
  • Monitors CSW ability to identify client preferences, capabilities, and limitations, without under stimulating the client
  • Communication skills (client, caregivers, co-workers, volunteers, supervisor)
  • reporting skills (changes in client condition, client preference, family updates, activity effectiveness) both verbal and written
  • Ability to change approach based on client needs
  • Active listening skills

 Occupational Health and Safety

  • Ensures that health and safety hazards and accidents are reported immediately and in compliance with Canadian Red Cross’ established policies and procedures
  • Investigates and thoroughly documents all accidents and reports, all time-sensitive documentation to the Occupational Health and Safety Department and Workplace Safety and Insurance Board
  • Creates and promotes a rehabilitation / return to work plan for injured workers that includes modified work assignments
  • Completes ongoing monitoring of the established rehabilitation / return to work plan
  • Complies with and understands the Occupation Health and Safety legislation and practices


  • Provides assistance and advise to the appropriate Accounting Department with regard to: record of employment requests, late timesheet follow-up, and client billing
  • Conducts routine CSW timesheet and travel audits to ensure compliance
  • Works within allocated budgets, monitors variances, and provides recommendations to remedy variances

Internal / External Communication

  • Develops and strengthens a positive working relationship with clients, funder, and other service providers
  • Actively participates in internal and external committees and community functions (i.e. workshops, job / health fairs, client conferences, provider meetings, participation on focus groups)
  • Continually improves professional development
  • Contributes to a healthy and safe working environment


The Minimum qualifications for this job are specialized post secondary education, RN or RPN  and between 3-6 years of job related experience, or an equivalent combination of education and experience.

  • Ability to assess effectiveness of programming
  • Excellent verbal and written communication skills
  • Thorough organizational, interpersonal and teaching skills
  • Experience with care coordination or case management is an asset
  • Experience in human resources management is an asset
  • Experience in a unionized environment is an asset
  • Working knowledge of MS Office software application
  • Knowledge of the Client Information Management System software is an asset
  • Ability to work non-traditional 
  • Bilingualism (Cree / English) is required
