Description

The Long-Term Care (LTC) Hemodialysis Nurse will be part of a new Hemodialysis program. This new clinical model of care is an exciting opportunity and will allow a group of patients to receive Hemodialysis care at their long-term care facility. The nurse will work onsite at the LTC facility in a renal clinic type of setting. The nurse will initiate, closely monitor and disconnect hemodialysis treatments while working collaboratively with both hospital and LTC. The Nurse will provide a consistent approach to promoting independent patient care as the patients advocate and leverage evidence-based practice to ensure the delivery of dialysis according to the plan of care.

REQUIREMENTS:

  • Registered with the College of Nurses of Ontario and in good standing
  • Previous Working Experience in one or more practice settings: home and community care, acute care, long-term care. 2 years’ experience is preferred.
  • Additional knowledge, training or experience in of Nephrology/Dialysis is preferred
  • Additional knowledge, training or experience in Central Vascular Access Device (CVAD) management is preferred
  • A current CPR certificate is required.
  • Proven professionalism, accountability, responsibility and reliability.
  • An aptitude for learning new skills and responsibilities.
  • Availability to work 8-10 hour shifts 2, 3 or 4 times a week.
  • A willingness to attend and complete mandatory training.
  • Proficient computer/technological skills and an aptitude for manual dexterity.
  • Able to work independently as well as part of an interdisciplinary team.
  • Well-developed interpersonal skills.
  • Maintains positive relationships to deliver person/family centered care.
  • Excellent communication skills, both oral and written, with the ability to escalate issues.
  • Excellent organizational skills.
  • Effective problem-solving and conflict management skills.
  • Remains flexible and works well in a changing environment.
  • A demonstrated commitment for Quality Improvement processes.
  • Must have reliable transportation.
  • A willingness to travel and attend meetings to support the needs of the patient.
