We are seeking a dynamic, passionate Registered Nurses (RNs), who wants to be part of the transformational change in home and community care by providing care in a truly client-centered approach. You will be part of a diverse and collaborative care team that will provide services to a roster of clients in the community. You will use evidence-informed nursing knowledge, skill, and judgment and work within the RN scope of practice to provide holistic nursing care to meet the physical, safety, emotional, and spiritual needs of clients and their families in their homes and communities.
As a member of our Wound/Ostomy team, you will promote excellence in evidence based clinical care for clients who have wound, ostomy or continence problems. Working with the local leadership team and supported by an Advanced Clinical Consultant, you will deliver standardized education, consultation and care, as well as participate in synergetic knowledge building with Wound and Ostomy Resource Nurses, colleagues and experts across Ontario. In addition, you will also conduct in-depth clinical and health assessments and provide consultation for clients requiring specialty wound management, ostomy and continence assessments.
