Job Summary:   Responsible for the direct supervision of the Community Health Worker.  Ensure the efficient and effective delivery of service to clients.

Key Duties and Responsibilities:

  • Provides guidance to service co-ordination to ensure effective day to day operations
  • Leads team meetings
  • Maintains and fosters effective and timely communication with schedulers and CHWs
  • Role models leadership skills that staff members should be developing
  • Practices Performance Management
  • Identifies skills and/or knowledge gaps relative to the Community Health Worker program operations and ensure the staff have the skill set to meet individual client needs
  • Provides opportunities and supports staff education and staff development
  • Participates in orientation, training and setting of performance expectations of new Community Health Worker staff
  • Ensures probationary and annual performance reviews of all Community Health Worker staff
  • Utilizes projections in determining and forecasting resource needs
  • Collaborates with Service Coordinators in the cost effective scheduling of Community Health Worker staff recognizing the impact of key cost drivers i.e. travel time, mileage
  • Facilitates the transfer of delegated skills
  • Follows employee relations processes and relevant policies and procedures
  • Inputs client/staff compliments, complaints and incidents into the Feedback/Risk Monitor
  • Investigates, follows up and responds to client/staff incidents and complaints
  • Completes data collection for chart audit and other quality reports
  • Conducts home visits for the purpose of care plan development, staff supervision, staff development, complaint/incident investigation or their client/staff issues 
  • All other reasonable duties as assigned


  • A Registered Nurse designation (CRNBC)
  • Previous community health care experience is required
  • A current CPR certificate is required
  • Excellent interpersonal and written and oral communication skills are required
  • Excellent time management skills and ability to work in a fast paced environment are required
  • Excellent problem-solving skills are required
  • An ability to work independently and as a team is required
  • A class 5 provincial driver’s license and a vehicle with business insurance are required
  • A willingness to travel is required
  • Flexibility to be on-call and to work days, evenings, and weekends is required
  • Basic keyboarding skills are required
  • Performance management experience is preferred
  • Familiarity with the funding and administration of community health services is preferred
  • Experience in the design and/or delivery of adult learning activities is preferred
