We are located in Vancouver and we provide home care services to the Vancouver, Burnaby and Fraser Valley communities. We are excited to announce that we are currently looking for CHWs to work in all of these areas on a casual basis.

RESPONSIBILITIES:

  • Provides home support services to clients such as assisting with activities of daily living, performing delegated tasks for which transfer of function training has been completed, planning and preparing meals, and light housekeeping.

DUTIES:

  • Assists clients with activities of daily living such as feeding, lifts and transfers, bathing, skin care, oral hygiene and toileting.

  • Performs delegated tasks for which transfer of function training has been completed, such as catheter care, suppositories, applying non-sterile dressings and implementing exercise and mobilization routines.

  • Administers medication to clients and provides medication reminders, in accordance with established policy.

  • Plans, prepares and serves meals and shops for groceries.

  • Observes clients and their environments and reports unsafe conditions and behavioral, physical, and/or cognitive changes to the client service coordinator or LPN supervisor.

  • Demonstrates methods and provides basic information to clients in relation to activities of daily living, meal planning and preparation, grocery shopping in accordance with pre-established care plans.

  • Completes and maintains related records and documentation such as communication books and progress reports.

  • Accompanies clients on outings such as appointments, shopping and leisure activities.

  • Performs other related duties as assigned.


QUALIFICATIONS:

  • Grade 12

  • Health Care Aid / Resident Care Aid Certificate

  • BC Care Aide registry number

  • Class V BC Driver’s License and access to a vehicle

  • OR an equivalent combination of education, training and experience

  • OR other Qualifications determined to be reasonable and relevant to the level of work

  • Flexible working hours, able to work evenings and weekends

    Home management skills

  • Physical ability to carry out the duties of the position

  • Ability to work independently and in cooperation with others

  • Ability to operate related equipment

  • Ability to communicate effectively both verbally and in writing

  • Ability to organize and prioritize

  • Ability to observe and recognize changes in clients

  • Ability to establish and maintain good rapport with clients and colleague
