Title : Community Health Coordinator

Employee Working Location : In-person (Kentville, NS)

Employment Status : Temporary Part-time (12 months)

Salary Band : $ 21,090 - $ 26,363 / year

In this role, you will :

  • Work with the Occupational Therapist to coordinate the day-to-day delivery of the Health Equipment Loan Program including the Specialized Equipment Program and Transitional Care Unit pilot expansion. Support with cleaning of the equipment as needed and service center duties under the direction of the onsite Service Center Coordinator as needed.
  • Liaise and build relationships with the hospital and long-term care for the overall delivery of the Health Equipment Loan Program (including delivery and pickup of equipment).
  • Maintain inventory records on equipment loans, returns and statistics in accordance with program policies.
  • Coordinate administrative functions such as ensuring financial transactions are completed and all information flow to public is timely and accurate.

         

What we are looking for :

  • The minimum qualifications for this job are completion of post-secondary education or diploma in a health-related field with one to two years of job-related work experience or an equivalent combination of education and experience in health, customer service, and administration.
  • Background as Occupational Therapy Assistant or Rehabilitation Assistant is an asset.
  • Ability to prioritize and organize multiple issues simultaneously in a complex client-based environment.
  • Sound decision making abilities, combined with strong organizational skills, are essential.
  • Strong communication skills, both written and oral, in English is mandatory.
  • Demonstrated computer proficiency and administrative skills.
  • Previous experience working with volunteers and seniors considered an asset.
  • Ability to do medium to heavy lifting.

 

Working conditions :

  • Valid driver’s license to travel within Nova Scotia and a satisfactory driver’s abstract is required.
