In this role, you will :

Responsibility 1. Coordinate, implement and evaluate the delivery of Community Health and Wellness programs within the community served.

  • Ensures the establishments of an effective plan of service / care which is consistent with MOH Policy and the Ontario Health MSAA, utilizing the InterRAI CHA assessment tool and includes the client and caregiver in care and service planning.
  • Completes InterRAI CHA assessments based on outcome scales and CCIM best practices, and as needed for each client (i.e., when the client’s or caregiver’s status or health condition changes), ensuring the Client Assessment Protocols (CAPS) inform changes in the client’s care plan, including client and caregiver input.
  • Assists the Manager by providing narratives around statistical data for Ontario Health reports and effective management as required.
  • Investigates incidents and fall events, completes reports, develops and implements prevention plans and modifies the service plan as required.
  • Actively engages the Community Support Workers (CSWs) in the development / revision of the care plan, recognizing that they are integral members of the client’s circle of care.
  • Collaborates with Home and Community Care to determine new client admissions to the program, receives, reviews referrals, and assessments from HCC, ensuring the established inter-agency business process are being followed.
  • Ensures provision of client personal support requirements from the individual’s care plan, including but not limited to:
    • Ensures meals meet client special diet requirements and requirements of Health Canada’s Food Guide.
    • Ensures safe food handling and storage practices are followed.
    • Ensures safe client medication storage.
  • Maintains current, accurate comprehensive documentation on all client records (in-home, electronic, office).

Responsibility 2. Human Resources

  • Trains and mentors staff on the program implementation and special functions.
  • Monitors Community Support Worker (CSW) performance by on site evaluation, internal / external feedback and client / caregiver feedback and brings forward to Manager/designate.
  • Receives and responds to CSW work-related concerns / issues in a timely manner, brings forward to manager/designate as required.
  • Monitors CSW ability to identify client preferences, capabilities, and limitations, without under stimulating the client.
  • Ensures that CSWs are informed in a timely manner of any potential or real risks within the scope of their position.

Responsibility 3. Internal/External Communication

  • Develops and strengthens a positive working relationship with clients, and other service providers.
  • Actively participates in internal and external committees and community functions (i.e., workshops, job / health fairs, client conferences, provider meetings, participation on focus groups).
  • Liaise with other community agencies/organizations regarding related services/initiatives to foster mutual understanding, coordination and partnerships.

         

What we are looking for :

  • The minimum qualifications for this job are specialized post-secondary education (RPN) and one year of job-related work experience, or an equivalent combination of education and experience.
  • Current Registered Practical Nurse with a current Certificate of Registration from the College of Nurses of Ontario.
  • Experience with care coordination or case management is an asset.
  • Excellent teaching and training skills.
  • Thorough knowledge of community health care, community programs/services.
  • Must be available to work on-call or non-traditional hour, if required.
  • Ability to implement a holistic approach to care.
  • Ability to engage the caregivers in care planning.
  • Ability to engage the client in care planning.
  • Ability to assess effectiveness of programming.
  • Working knowledge of MS Office software application.
  • Excellent verbal and written communication skills in English. French and Cree are considered an asset.
