Title: Program Coordinator - PATH (Priority Assistance to Transfer Home)  

Employee Working Location : In-person (North Bay, Ontario)  

Employment status : Temporary full time (12 months)

The incumbent will coordinate and implement all aspects of the Priority Assistance to Transfer Home (PATH) program in collaboration with Team Lead. The PATH Program is a community program designed to facilitate a smooth transition from hospital to home and ensures that community support services are in place for clients.  
   

In this role, you will:                                                                                         

  • Develop, coordinate, implement and evaluate the delivery of the PATH program.  
  • Participate in client discharge planning (hospitals/community support services).  
  • Approve referrals, complete intakes, and determine internal or external care coordination.  
  • Schedule request for service/ transportation and confirm referral acceptance back to the participating hospital.  
  • Ensure staff follow safety protocols and policies of the agency and stakeholders.  
  • Follow up with PATH clients within 48 hours after service delivery to complete the service, assess for additional services, refer to other services as appropriate , and conduct client satisfaction surveys.  
  • Assist with preparing and monitoring program specific budgets, complete program statistical reports and assist with preparing progress reports.  
  • Establish the qualitative and quantitative data required to evaluate the program in consultation with hospital, HCC, and participating agencies.  
  • Play an active role in identifying , developing, and implementing new or re-aligned services/initiatives.  
       

What we are looking for:  

  • A Bachelor’s degree or one to three years job related experience or/and  an equivalent combination of education and experience.   
  • Fluency in English is required, French is an asset  
  • Valid drivers license and access to a reliable vehicle with the ability to travel in the community.  
  • Proficiency in Microsoft Office applications (Word, Outlook, Excel, Power Point) is required .  
  • Proven experience in program management and development.  
  • Good communication /presentation skills, leadership abilities, and experience working with clients and volunteers is essential.  
  • Experience with administrative tasks such as reporting, documentation, scheduling calls  
  • Standard First Aid and CPR-C training is an asset    
  • Knowledge of community support services  

 

Working Conditions:  

  • The majority of   the work will be performed in a clean and comfortable office environment, however travel throughout the assigned geography may also be required .
